Staff Training Dates for 2016:

The Surgery and Dispensary will close each month for staff training between 2pm and 4pm.  Cover for EMERGENCIES ONLY will be provided by the Out of Hours service.

Do It Online

We offer online patient access to:

  • Appointment booking
  • Repeat prescription requesting
  • Access to view of patient record
  • Access to detailed record

To register for Online Access:

  • You must attend the surgery in person and ask for the appropriate forms at Reception or Dispensary.
  • Once your identity has been verified (two documents need to be presented for your identity to be verified, at least one of which should contain a photo of the patient. Any two of the following three documents are acceptable: passport, driving licence, bank statement) a document will be printed with the log-on details and instructions to enable you to complete your registration online at home.

  • There is a 2 week expiration date on the registration details document. If the registration is not completed in this time you will need to ask for your Patient Access to be re-set and another registration letter printed so you can try again.

Online Access on behalf of another registered patient:

Parents/carers can request online access for a patient registered here. You will need to complete and return an additional form before proxy access can be given. These forms are available from Reception, Dispensary or the website.

Children under 11 years:
  • Online access for managing repeat prescriptions and appointments can be given to a parent/carer.
  • Parent/carer access will be removed when the patient reaches the age of 11.

Children aged 11 to 15 years:
  • Online access for managing repeat prescriptions and appointments can be requested by the parent/carer with the child's consent.
  • The child can also request online access with parental consent.
  • Access can also be given to the child without parental consent at the discretion of his/her GP - please note this GP has to be familiar with the child in order to grant this request.

Proxy Access - Consent for 11 - 15 year olds 

Both parties must sign the form in the presence of surgery staff.

 
Over the age of 16:
  • Online access for managing repeat prescriptions and appointments can be requested by a Carer with the patient's consent
  • Access can also be given to a Carer at the discretion of his/her GP, if the patient lacks capacity. Please note: this GP has to be familiar with the patient in order to grant this request

Terms and Conditions of Use

  • It is your responsibility to ensure that your Online Access details are not used by anyone else.
  • The practice does not have access to your password. If you have forgotten your password, we cannot reset it for you. We can only cancel your registration and issue a new sign-up document so that you can re-register.
  • We will be monitoring the use of this service.
  • If we find that a user is abusing the service, we will issue a warning letter.
  • Where the situation does not improve, or recurs, access will be removed.
  • Examples of inappropriate use are:
    • booking appointments and not turning up for them
    • booking appointments for other family members using your name
    • consistently booking inappropriate appointments with the doctor or nurse.
  • The message box on the Prescriptions page should only be used for comments relating to your selected prescriptions.
  • We cannot action requests for medication that is not on your repeat list, nor reply to any questions raised within this message box
