We would like to let you know about a change to our recall system for Long Term Conditions such as Diabetes, Asthma and COPD.
Starting this month we will be inviting patients with the above conditions during their birth month. This offers a more comprehensive approach to Long Term Conditions reviews and invite follow ups.
We appreciate some patients may have been reviewed for their long term conditions in the last couple of months – If this is the case, please ignore the invite when your receive it or if you are unsure please call our reception team.
